LDL-lowering Independent Effects of Early Pre-treatment with High-dose Statins in Patients Undergoing Percutaneous Coronary Interventions

چکیده
Statins inhibit hepatic 3-hydroxy-3-methylglutaryl Coenzyme A (HMG-CoA) reductase with consequent suppression of cholesterol biosynthesis. The advent of these drugs has significantly impacted the treatment of cardiovascular disease with well documented benefit in primary and secondary prevention1-3. However, several clinical and basic science investigations suggested that the beneficial effects of statins may extend beyond their cholesterol lowering effects, the so-called “pleiotropic effects”; these include improving endothelial function, inhibiting vascular inflammation and oxidation4-7. Pre-Procedural Atorvastatin Mobilizes Endothelial Progenitor Cells: Clues to the Salutary Effects of Statins on Healing of Stented Human Arteries
OBJECTIVES Recent clinical trials suggest an LDL-independent superiority of intensive statin therapy in reducing target vessel revascularization and peri-procedural myocardial infarctions in patients who undergo percutaneous coronary interventions (PCI). While animal studies demonstrate that statins mobilize endothelial progenitor cells (EPCs) which can enhance arterial repair and attenuate neo...
متن کاملChronic pre-treatment of statins is associated with the reduction of the no-reflow phenomenon in the patients with reperfused acute myocardial infarction.
AIMS Early statin treatment has beneficial effects on prognosis after acute coronary syndrome. The no-reflow phenomenon determines the prognosis after acute myocardial infarction. We investigated the effects of statin treatment before admission on the development of the no-reflow after infarction. METHODS AND RESULTS We performed intracoronary myocardial contrast echocardiography in 293 conse...
متن کاملAre pleiotropic effects of statins real?
The clinical benefits of statins are strongly related to their low density lipoprotein-cholesterol (LDL-C) lowering properties. However, because mevalonic acid (MVA), the product of 3-hydroxy-3-methyl-3-glutaryl coenzyme A (HMG-CoA) reductase reaction, is the precursor not only of cholesterol but also of nonsteroidal isoprenoid compounds, the inhibition of HMG-CoA reductase may result in pleiot...
متن کاملRelation of inflammation and benefit of statins after percutaneous coronary interventions.
BACKGROUND Beyond lipid lowering, statins are known to possess antiinflammatory and antithrombotic properties. Recent studies suggested an association between statins and early reduction in death or myocardial infarction (MI) after percutaneous coronary interventions (PCIs). We sought to examine the interrelationship between inflammation, statin use, and PCI outcomes. METHODS AND RESULTS In t...
متن کاملStatins and their role in pre-percutaneous coronary intervention.
Lipid-lowering therapy with statins reduces the risk of cardiovascular events in patients with coronary artery disease. Recent in vitro and in vivo studies demonstrated a low-density lipoprotein-independent action of this class of drugs, which appears to modulate endothelial function, inflammation, and thrombosis. Randomized studies showed a beneficial effect of short-term statin pretreatment i...
